It's a late 17th/early 18th century world with late 17th/early 18th century medical technology. So either Louis was always a biological girl, or magic is involved.
Or just the way she’s dressed up.
The character list says Katherine Louise Steward. So Louis is a nickname based on her middle name.
This will be explained later in the flashback.

A serie about a guard of Louis XV of France who is possesed for his sister assasinated and who guided him and three friends (a lover not recipal from her, the master who teach swordmanship to him adn a servant child of the court) on the search of her assasination and a terrible complot who involved Europe.
It's based in a french's spy of that age who was named like a man, spyed like man or woman and the rest of life live like a woman
It's an anime related to a woman who was socially treated and acknowledged as a man and asked to be identified as a woman.

She argued she was born a woman, but her father (Louis interestingly enough) changed her gender into a man later on. Autopsy revealed male organs, though notable feminine traits. Meaning she was either inter-sex (this is the most probable, since it would mean no one is lying, there just wasn't a good understanding of the biology of sex at the time) or a transwoman, at least assuming the story of surgeon was true.
"Or magic is involved" is always possible, despite most of it being drained off in the aftermath of the whole Raana affair. And it would be a worthy application to help someone's body match their identity.
I suspect Louise may still suffer from prejudices, but if there's one thing I have learned it is that Rich's stories have one uniting feature: compassion.

I knew there had to be -some- connection to louis, because the chapter we met him was entitled the boy, but this is even more fantabulous than i expected! Brava!

But you picked up on an actual clue, something that was and is part of the character and storyline. Mine was simply a feeling based on something ephemeral, and my thought only that the woman was related\connected to Louis. Not that it would BE Louis. So well done for your conclusion 😁

Yeah, the related strips have some vague algorithm that nobody understands. Usually at least 1 character will be in both, but not always, and it's unclear why that particular strip is chosen.

I adore that The Woman still has her hands on his shoulders, fully trusting his acceptance. Mmwah! (Big mushy chefs kiss noise). (And it's especially marvelous after Charles flinched in the previous panel from the exuberant greeting from somebody he doesn't realize he knows.)
In fact, the greeting was so exuberant that Charles dropped his hat.
I am less shocked by the change from Louis to Louise than the change of handedness. (Of course, this may be leading up to, "Why are you smiling?" "Because I know something you don't know." And what is that?" "I'm not left handed!")
